Registration

New Patients

The practice area consists of parts of SE5, SE24 and SE22. Patients who live in our practice area may register with our practice. If you would like to see a particular doctor, please inform the receptionist and we will do our best to accommodate you. Please ring us for more details, or use our online service.

It is very important that you notify us of any changes in name and address. Please note that if you move our of our practice area, it will be necessary for you to register with another GP locally. A map of our area is displayed at reception.

All newly registered patients are offered a health check on registering with the practice.

Change of Personal Details

If you change your name address or telephone number please remember to give the details to reception, or use our online services in order that your records together with and any other member of your household can be amended. This is most important, as the doctor may need to contact you in the future.
